Observation

110-300-0165(2)(a): Tamper-resistant (TR) covers were observed plugged into TR outlets in the bathrooms and some classrooms. Plastic bags were observed in the upstairs classroom at a level accessible to children, and110-300-0165(2)(d): in bathroom cabinets under sinks, which did not have safety locks installed. Cords were accessible to children in several classrooms. Devices that were in use were110-300-0165(2)(c): not properly stored after use, and humidifiers were placed in some classrooms with cords sticking out forming a loop. Some outlets in the classrooms were not TR outlets, but had TR covers plugged in,110-300-0165(5)(a): which could pose a choking hazard.
